Program: SoCal String Academy Summer Institute
Location: iDucation Center, Chino Hills, CA
Dates: Summer 2026 Camp Sessions (June–July 2026)

By enrolling my child in the SoCal String Academy Summer Institute, I acknowledge and agree to the following:

1. Acknowledgment of Risk

I understand that participation in camp activities—including but not limited to violin instruction, movement activities, use of musical instruments, art projects, and group play—may involve certain inherent risks. These may include minor injuries such as falls, bumps, or allergic reactions to materials.

I voluntarily assume all such risks on behalf of my child.

2. Medical Authorization

In the event of an emergency, I authorize SoCal String Academy staff to obtain basic medical care for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

3. Release of Liability

I hereby release and hold harmless SoCal String Academy, its owner, instructors, staff, and facility partners from any and all claims, injuries, damages, or liabilities arising from my child’s participation in the program, except in cases of gross negligence or willful misconduct.

4. Health & Safety

I confirm that my child is in good health and able to participate in camp activities. I agree to keep my child home if they are ill and to inform the academy of any relevant medical conditions, allergies, or special needs.

5. Personal Property

I understand that SoCal String Academy is not responsible for lost, stolen, or damaged personal belongings brought to camp.

6. Agreement

I have read and understand this Liability Waiver & Release Agreement. By completing registration, I agree to these terms voluntarily and acknowledge that this agreement is binding.
